acial implants and fat grafting are two ways to serve the same goal: increasing volume in the face. We lack volume in the face for two main reasons: loss of volume due to aging (mostly fat), and congenital (bony and soft tissue deficits). We see these manifest visually as hollow, sunken cheeks and eyes, or a small, recessed chin or thin lips. There are a variety of ways to address these concerns depending on the cause of the volume loss which includes fat grafting (particularly for the cheeks) and implants (very effective for weak chins).

Chin implantation is typically performed with medical grade pre-formed silicone implants. There are a variety of implant shapes and sizes that can be used to address your specific aesthetic concern. For the more cautious patient, injectable fillers such as Radiesse can be used for temporary chin augmentation. Implants are usually placed through either an intraoral or submental incision. There are benefits and drawbacks to both techniques, but both are safe and effective ways of placing silicone mandibular implants. Fat is obtained from areas of excess (usually the abdomen or another body area), purified, then injected to the face in areas of volume deficit. Not meant to correct fine lines.

30-60 minutes depending on the degree of correction required

What is the recovery process like?

Majority of swelling and bruising resolved in 3 weeks, with normal mild swelling that can take up to 3 months to resolve. Incisions (if used) usually slightly red for at least 4 weeks

How soon can I return to work?

Most patients can resume non-physical work in 2-3 days

How soon can I return to my regular exercise routine?

No lifting more than 20 pounds, and avoid bending/straining for 1 week

Will follow up appointments be necessary?

Usually 1,4, and 12 weeks after surgery

What can be accomplished with fat grafting and implants?

The goal of fat grafting is to restore lost volume in the face. Loss of volume can cause the face to look aged, tired, and deflated. Targeted fat grafting to areas of volume loss can help you regain your youthful facial contours, soften wrinkles, and even lift the skin. Often patients that come in for consult initially believe they will need a facelift, but rather than sagging of the skin what they actually have is volume loss which can be addressed with fat grafting.

Implants such as those for the mandible (chin) can help improve the anterior projection, reducing the appearance of a “weak” chin and significantly improving facial proportions and balance. Implants can also be used to augment the lips for fuller lips, although autologous tissue and fillers can be used as well.

What type of anesthesia is used for this procedure?

Facial fat grafting and most implants can be performed under local anesthesia, depending on the ability of the patient to tolerate it. Fat grafting is often done in conjunction with other procedures such as facelift which may require IV sedation or general anesthesia. Your doctor with help you determine what is the right type of anesthesia for your specific situation.

What is the recovery like?

There can often be noticeable swelling and bruising after facial implants or fat grafting. These usually dissipate over the course of a week or two. Pain is usually manageable with over the counter medications although stronger medications are often prescribed. You will be able to return to work as soon as you feel comfortable doing so.
